In 2007, ElderChoice launched Choices For Change, which continues to be an approved Structured Adult Day Program for the Traumatic Brain Injury (TBI) and Nursing Home Transition and Diversion (NHTD) Waivers in Cayuga and Onondaga counties. We are also currently approved as a Social Adult Day Care program through the New York State Office for the Aging and have developed and maintained an engaging, positive social environment for all our participants. This valuable service is offered through several of our programs, including the TBI and NHTD Waivers, Managed Long Term Care (MLTC) Plans, as well as to private pay participants.

What is Social Adult Day and Structured Day Program?
Structured Day Program (SDP) services are specific to the TBI and NHTD Waiver programs and are specially designed services provided in a community setting which are intended to improve or maintain the individual’s skills and ability to live as independently as possible in their home and/or community. This service is provided by professional staff (at least 1 Personal Care Aide or Home Health Aide) Services include an initial assessment which is used to create a personalized goal plan for each individual. Day Program staff provide participants with training and supervision related to activities of daily living (ADLs), and many other valuable life skills including but not limited to: task completion, communication and interpersonal skills, problem-solving, socialization, sensory/motor skills, mobility, community transportation, money management skills and tasks relating to maintaining a household.
Social Adult Day Services (SADS) is a structured and inclusive program that provides functionally impaired adults (those who may need assistance with various activities of daily living) with a wide range of services in a safe setting. Our program offers 2 options: either 1- 4 hours or 4-6 hours Monday – Friday. Every social day program participant receives personalized services based on a service plan that is unique to them and that is based on an assessment of the individual.
Participants of both can enjoy the following activities: Nintendo Wii; Weekly Community Outings; Fitness & Health activities; Tuesday’s “Cooking with Me”Classes; Computer Literacy; Socialization & Group Activities; And Much More!!
