What is the Nursing Home Transition & Diversion Waiver?
The NHTD Waiver is a NYS Medicaid funded program, specifically designed for adults who have sustained a physical disability, have a chronic illness and/or are over the age of 65, who require supports to circumvent a nursing home or institutional level of care. Services through the NHTD Waiver are individually designed to provide support for person-centered goals, while ensuring the participant can reside safely in their own home.
A participant in the NHTD Waiver may choose to receive Service Coordination only, or a combination of Service Coordination plus ancillary services to support their health, welfare and overall well-being.
Referrals to the NHTD Waiver Program must be made through the local Regional Resource Development Center (RRDC), as directed by NYS Department of Health.

Who is eligible?
To be eligible for enrollment in the program, you must be:
- 18 years to 64 years with a physical disability or chronic illness
- 65 years of age or older
- eligible for nursing home level of care
- have active Medicaid or be eligible for Medicaid coverage for community-based long term care.
ElderChoice has provided exceptional services to participants in the NHTD Waiver program, since the programs inception!
Currently we offer the following NHTD Services:
Home and Community Support Staff (HCSS)
Home and Community Support Staff assist the participant with activities of daily living, both in their home and community. These tasks can include, but are not limited to bathing, dressing, light housekeeping, verbal prompts for self- medication administration, meal preparation and oversight/supervision. This service is provided in hourly increments and can be approved for up to 24 hours/day, if necessary.
All HCSS are certified Personal Care Aides.
Structured Day Program (SDP)
Structured Day Program provides participants the opportunity to socialize, re-integrate into their community and develop additional leisure and recreational skills in a supported, group environment.
